Am I eligible to take part in the ReCAPS Study?

To confirm if you are eligible to take part in the ReCAPS study, please complete the following forms:

Once one of the research team has had a chance to review the information, they will contact you directly.

Please note that these questionnaires contain questions about your medical history and other personal information that may be sensitive. The questions are necessary to confirm whether you are eligible to take part in the study. The information you provide will remain confidential and only be viewed by members of the ReCAPS Study research team.
